AsianPacificJCancerPrev,10,1147-1150，2009【Abstract】Objectives:Toevaluatetheefficacyandsafetyofaweeklytaxanescheduleinthetreatmentofadvancednonsmallcelllungcancers(NSCLCs)andtogenerateanoptimalpre-medicationprotocolforweeklytaxane.Methods:FromDecember2001toJune2006,78patientswithadvancedNSCLCswererecruitedfromtheDepartmentofChemotherapy,JiangsuCancerHospitalandResearchInstitute.Paclitaxelwasdeliveredat80-100mg/m2ondays1and8(11cases),or50-80mg/m2ondays1,8and15(23cases),whiledocetaxelwasgivenwiththesameschedulesat35-45mg/m2(30cases),or25-35mg/m2(14cases).Inallcasesthiswascombinedwithaplatinumbaseddrug(cisplatin,oxaliplatinorcarboplatin),followedbya1weekrest.Fourpre-medicationswereattemptedwerealsocompared.Results:All78patientsreceivedatotalof202coursesoftreatment.Doselimitingtoxicitywasmyelosuppression.Grades3and4leukopeniaoccurredin19.2%(15/78).Ofthe56eligiblepatientswhocompletedatleast2courses,nonehadacompleteresponse,20achievedapartialresponseand5showedprogression.Toxicityofpre-medicationswasindicatedby:hypersensitivity(1case),hypopotassemia(8cases),myasthenia(5cases),hiccups(1case)andinfection(2cases).Notreatmentrelateddeathsoccurred.Conclusions:Weeklyadministrationofpaclitaxel/docetaxelisasafeandactiveprotocolforadvancedNSCLCs.Ourrecommendationsforweeklypre-medicationwithtaxaneare:dexamethasone2.25mg-7.5mgorally12hand2hbefore,promethazineandcimetidine30minbeforepaclitaxel;
oraldexamethasone4.5mg-7.5mgtwicedailyforthreeconsecutivedays(thedaybefore,thedayof,andthedayafterdocetaxel),promethazineandcimetidine30minbeforedocetaxel.
